I think there's a particular kind of creepiness- normality suddenly revealed to be something horrific- that's captured here. The smile is what sells it. When I was little, I'd have nightmares where something familiar, like an aunt or my house, would turn into evil beings. It was worse if they smiled, That meant that the they knew they were evil, and enjoyed it. It also meant they won.
I had a very similar feeling when I came across this image. I had been browsing through more mundane art when this gem popped out at me. I didn't know DA could make me feel that way.
